Abstract
A case of successful treatment of a rare pathology due to aneurysm in the distal part of the ulnar
artery or in the ulnar part of the palmar arch is presented. Genesis of such aneurysms is
described as hypothenar hammer syndrome which is characterized by different types of
ischemia in hand tissues. The cause for it is the disturbance of circulation due to thromboembolic
complications. There are different views on the conservative or surgical treatment of the
aneurysm in the distal part of ulnar artery. Resection of the true aneurysm in the ulnar part of
the palmar arch with formation of end-to-end microvascular anastomosis was performed in the
given case. Consequently, the aneurysm was removed, the possibilities of its rupture and further
thromboembolic complications were eliminated. Restoration of ulnar arch circularity in a
young patient allowed decreasing the risk of ischemic complications development in different
critical cases.
