Abstract
Aortic valve replacement is the current standard of care, but is more complex in the context of coronary artery bypass grafting. Despite such acceptable results, the mortality rate decreased from 18 to 0.7%, while performing aortic valve prosthetics after a previous coronary artery bypass surgery, unresolved issues remain. Additional operational risks are associated with the presence of adhesions, the complexity of sternotomy, the possibility of damage to functioning shunts, the need for adequate cardioplegia and clamping of the aorta. For this group of patients, careful planning of the primary coronary artery bypass surgery with regard to possible future sternotomy and aortotomy is extremely important. These difficulties require further analysis in order to find optimal mechanisms for their solution, taking into account modern technologies and new surgical approaches in the treatment of severe aortic stenosis in patients after coronary artery bypass grafting.
