Abstract
Introduction. The formation of adhesions remains one of the dangerous complications that occur in surgery, including in cardiac surgery. This problem is very urgent and far from being resolved. Films and plates (in comparison with gels and solutions) based on biodegradable polymers may be the most promising in solving this problem. Animal gelatin is widely used in medicine. From the point of view of biological safety gelatin from fish raw materials may also be promising. Fish gelatin is a relatively new version of gelatin, which has shown the promise of its use in agriculture and the food industry. The purpose of this work is a comparative assessment of film compositions from animal and fish gelatin in the prevention of postoperative adhesion formation.
Material and methods. Fish gelatin from fish scales of the Volga-Caspian basin (Astrakhan State Technical University, Astrakhan), and highly purified animal gelatin (LLC Mozhelit, Mogilev) were used as a raw material. The medicinal product is an anti-inflammatory herbal preparation olchicine/Kolkhikum-Dispert. The plasticizer is glycerin. The films were sterilized with ethylene oxide. Physical and mechanical characteristics, degradation time, and interaction of samples with native blood in vitro were evaluated. Experimental studies were conducted in vivo on pigs.
Results. Based on the literature data and our own assessment, it was found that the positive quality indicators of fish gelatin correspond to animal gelatin used in the medical industry: good water solubility, film formation, degradation to non-toxic substances, the possibility of sterilization with ethylene oxide, but unlike animal gelatin, fish gelatin is more biocompatible, has increased strength, greater elasticity, increased adhesion to biological tissues and better handling properties.
Conclusions. In vivo experiments on pigs, the effectiveness of film compositions based on fish gelatin in the prevention of adhesion in the pericardial cavity was confirmed.
