Abstract
Recently, due to the increasing introduction of percutaneous coronary interventions, surgical treatment of postinfarction left ventricular (LV) aneurysm is losing its relevance to a certain extent, but in spite of that it remains as an important problem because of the high risk of surgery-related complications and increased hospital mortality. Searching for the optimal methods to improve the immediate and long-term results of LV reconstruction is still an important problem. One of the key points in the reconstruction of the left ventricle, as in other cardiac surgical interventions, is an adequate protection of the myocardium, but in the surgery of postinfarction LV aneurysm myocardial protection is essential because of the severe pre-operative LV dysfunction. In patients with large aneurysms and reduced LV ejection fraction up to 30% adequate myocardial protection is key for a successful surgery.
