Abstract
Due to population ageing and increasing life expectancy, aortic stenosis (AS) has become the most common valvular heart disease. Transcatheter aortic valve implantation (TAVI) is currently indicated in elderly patients with severe symptomatic aortic stenosis at high surgical risk (STS-PROM/EuroSCORE II >8%). Thе comparative clinical outcomes of surgical aortic valve replacement (SAVR) and TAVI are provided in this paper. Despite being minimally invasive, TAVI is associated with plenty of complications including bleeding, stroke, vascular injury, arrhythmias, need for a pacemaker due to conduction disturbances, and infective endocarditis characterized by an extremely high mortality. This article summaries current data on the prevalence, risk factors, microbiological profile, clinical outcomes as well as diagnosis and management of infective endocarditis related to TAVI. A clinical case of SAVR after TAVI-IE is described in details. Besides, the main clinical, anatomical and procedural factors that influence the choice of treatment for AS are also presented.
