Abstract
Тhe number of patients with heart valve diseases in combination with multifocal atherosclerosis has been increasing in most countries of the world in the recent years, especially among the elderly and seniors. Elderly patients often have various concomitant chronic illnesses related to cardiovascular changes and aging processes, placing them in a high surgical risk group. To reduce the duration of the cardiopulmonary bypass, aortic compression, and the risk of surgical and postoperative complications in this patient group, a hybrid method of surgical treatment is widely used nowadays. It consists of a staged implementation of percutaneous endovascular intervention on coronary arteries and valve replacement under artificial circulatory system.
This case illustrates a favorable outcome of surgical correction for critical aortic stenosis, followed by subsequent myocardial revascularization, in a geriatric patient presenting with multifocal atherosclerosis. The study presents an 11- year follow-up, along with a comprehensive analysis of the patient's health-related quality of life. Our results demonstrate that undertaking such surgical procedures in geriatric patients with comorbidities is feasible and yields beneficial immediate and long-term results, significantly enhancing their overall quality of life.
