Abstract
Publications review is dedicated to one of the most topical contemporary problem - age aspects
of the surgical treatment for ischemic heart disease (IHD). Information of the main peculiarities
of surgical treatment for ischemic heart disease paying attention to the two most contradictory
groups: young patients (under 40 years) and patients of 70 years old and more was presented
based on the analysis of contemporary domestic and foreign literary sources.
In spite of several investigations results there are quite a number of unsolved and contradictory
issues, such as late surgical results and factors determining them (a group of young patients) and
advisability of coronary artery bypass grafting (CABG) in patients of 70 and older. Due to severe
coronary arteries and myocardial involvement a lot of concomitant diseases and great number of
women with acute coronary syndrome in the latter group surgical lethality rate is still high and
requires further studies to improve surgical results. Such issues as selection of the type of surgical
procedure in patients of senior age groups - coronary artery bypass grafting with cardiopulmonary
bypass (CPB) or procedures on the beating heart, advisability of complete or incomplete
myocardial revascularization, conduit selection and importance of autoarterial myocardial
revascularization are finally not solved.
