Abstract
Introduction. Lately, an increasing number of patients with ischemic heart disease (IHD) at
the age of 70 and older, has been noted in developed countries including Russia. Elderly
patients with IHD have the peculiarities which must be considered during the surgical treatment.
Moreover, the overall trend in management of such category of patients is aimed at surgical
treatment. The objective of our study was to evaluate the immediate results of surgical
treatment in this group of patients.
Material and methods. Immediate results were studied in 53 patients with IHD at the age of 70
years and older. Initial severity of patients clinical state, nature of postoperative complications,
main operative and mortality risk factors were analyzed.
Results. Significant severity of IHD clinical manifestations due to major coronary artery
lesion, high rate of concomitant diseases and multifocal atherosclerosis was noted. The most
common postoperative complications were: atrial fibrillation, cerebral edema, renal insufficiency.
Multivariate logistic regression model revealed that independent risk factors for mortality
and complications were: multifocal atherosclerosis (odds ratio 1.3; p=0.02) and renal
diseases (odds ratio 3.4; p=0.03). Operative mortality was 1.9% (1 patient).
Conclusion. Obtained findings allow for having an optimistic look on the problem of IHD surgical
treatment in patients at the age of 70 years and older despite the initial severity of
patients clinical state.
