Abstract
Potential candidates for the heart transplantation represent the most serious category of cardiac surgery patients.
Despite the evidence of improving prognosis in patients on account of medicament therapy which have been
reported in randomized researches the available now medicaments have had the unstable effect on patients being
in terminal stage of disease. In such situation the most optimal treatment tactics shall be the use of different systems
of auxiliary circulation as a bridge to transplantation because the donor heart is not available.
The report has described the clinical case as a successful example of implantation of ventricular assist device to the
patient needed the urgent heart transplantation. It is shown that the implantation of ventricular assist device in
this category of patients is an option method because of the absence of contraindications and fitted donor heart.
At the same time follow up the patients after implantation of ventricular assist device shall demand the use of anticoagulation
and antiplatelet therapy which increases the risk of hemorrhages in early postoperative period.
