Abstract
Introduction. The study analyzed the long-term results of coronary artery bypass grafting in patients with
CHD and diabetes to assess the predictive value of diabetes and to identify other risk factors.
Materials and methods. The study included 337 patients with multivessel coronary artery disease after CABG
spacecraft, divided into 2 groups: Group 1 included 195 patients with diabetes, group 2 included 142 patients
without diabetes (control). Analyzed all-cause mortality, rate of non-fatal myocardial infarction, non-fatal
stroke, the need for repeat revascularization, as well as the frequency of the set of all events (so-called "major"
cardiovascular complications). According to the results of the regression analysis were assessed value diabetes
and other possible risk factors adversely affecting the prognosis of the disease.
Results . In patients with diabetes incidence of total mortality, myocardial infarction, stroke, and the need for
repeat revascularization was significantly higher than in patients without diabetes. Diabetes mellitus is an
independent risk factor for long-term mortality (HR — 1.32, 95% CI 0.92—2.05, p = 0.02) and the development
of "large" cardiovascular events (death, myocardial infarction, stroke, and repeated revascularization)
(HR — 3.1, 95% CI 2.87 — 3.27, p = 0.002). Availability insulin-treated diabetes is more important significant
predictor: HR of death — 2.71 (p = 0.001), and the relative risk of "major" cardiovascular complications —
4.2 (p = 0.001). Other independent predictors of death are age, smoking, myocardial contractility and the
presence of COPD. Predictors of the "major" cardiovascular events, also became, dyslipidemia, cerebrovascular
atherosclerosis and chronic renal failure.
Conclusion. A long-term result of CABG in patients with diabetes is worse in comparison with patients without
diabetes. Type II diabetes mellitus is an independent risk factor for long-term mortality and "major" cardiovascular
events (death, myocardial infarction, stroke, repeat revascularization).
