Abstract
Atrial fibrillation is one of the most common form of arrhythmia, which complicates the mitral valve patho
logy. The formation of atrial thrombosis in presence of atrial fibrillation depends not only on the arrhythmia,
but also on the presence of accompanying organic disease of the heart. Atrial thrombosis with thromboem
bolic complications is a life-threatening complication and significantly increases morbidity and mortality
when valvular pathology and atrial fibrillation are combined. Coronary-cardiac fistulas can occur without
any symptoms and do not cause any complications in the first two decades of life but with age such compli
cations as steal syndrome, heart failure and atrial fibrillation can occur as a result of atrial volume overload,
atrial dilatation and myocardial ischemia.
We represent the rare clinical case of successful correction of a coronary-cardiac fistula with pathology of
mitral, aortic and tricuspid valves, complicated with atrial fibrillation and left atrial thrombosis and high pul
monary pressure. Our experience demonstrates the effectiveness of modified «Maze-3» procedure with use
of a cryoablation in combination with correction of the accompanying cardiac pathology.
