Abstract
Surgery of aneurysms of the ascending aorta remains one of the most difficult aspects of cardiac surgery. One
of the most severe group of patients with this pathology of the aortic root are patients with long current aortic
regurgitation caused annuloaortic ectasia of various etiologies. Overload capacity leads to pathological
remodeling, changing the geometry and functional status of the left ventricle, mitral valve (MV) insufficiency
development. Patients with a combination of an aneurysm of the ascending aorta and the left ventricle
dilatation constitute a significant high risk for development of heart failure in the postoperative period. This
report describes the experience of three cases of papillary muscles of the mitral valve approximation by PTFE
loop with prosthetic ascending aorta and aortic valve replacement. The method was used as an addition to
the reconstruction of the MV and the way to effect on left ventricular geometry. The results show a significant
improvement in the morphological characteristics after heart surgery in 24 months follow up.
