Abstract
Introduction. Rupture of the interventricular septum (VSD) is one of the most severe complications of acute myocardial infarction. The increased number of surviving patients after surgery helped initiate studies to evaluate long-term results of surgical treatment of postinfarction IVS break.
Aim. Evaluation of long-term results of surgical treatment of CHD patients with post-infarction ventricular septal rupture to address the issue of the effectiveness and feasibility of surgical treatment.
Material and methods. In this study, summed up the experience of surgical treatment of 46 patients with coronary heart disease c postinfarction rupture of the interventricular septum (IVS), operated in A.N. Bakulev Scientific Centre for Cardiovascular Surgery Health Ministry from 2001 to 2014. The average age of the group was 57.4±2.1 years (37 to 82 years). Long-term results of surgical treatment in patients with post-infarction interventricular septum rupture were studied in 38 patients within the period of 6 months to 12 years.
Results. Despite the increased hospital mortality rate (17,4%), surgery provides a clinical improvement in all surviving patients after surgery, which confirms the effectiveness of surgical treatment. Good long-term 5 and 10-year survival (85% and 77%) testifies to the high efficiency of reconstructive surgery in the complex treatment of patients with coronary artery disease with postinfarction IVS break. Poor stability of the clinical results for 10-year follow-up (23%). The main risk factors for complications are Patches recanalization in postinfarction rupture, myocardial contractility of the left ventricle, as well as the original multi-organ failure.
Conclusion. Based on the data, concluded that the high efficiency of surgical treatment in most patients with post-infarction ventricular septal rupture.
