Abstract
Atypical clinical picture, no significant signs of myocardial ischemia due to the favorable situation with
atherosclerosis risk factors often lead to late diagnosis of coronary atherosclerosis and coronary heart disease.
Such a situation often develops in women. The development of coronary heart disease in women has a number of features and is often associated with pathologic and pathophysiologic causes of circulatory disorders
of the heart muscle. So often in middle-aged women constrictive changes in the vessels appear in the distal
part of artery and at the level of microcirculation with the development of hemodynamic syndrome X.
However, atypical symptoms, probably, against the backdrop of compensatory processes may be at the proximal
coronary arteries. The presented case illustrates an atypical symptoms and difficult diagnosis in a patient
with stenosis of the left main coronary artery. Obviously, in such situations need a more accurate diagnosis
using modern imaging techniques. Off-pump myocardial revascularization is an effective method in patients
with stenosis of the left main coronary artery.
