Abstract
Currently, much attention is paid to the timely identification and correction of arterial hypertension. Great
strides have been made in the diagnosis and treatment of arterial hypertension of various etiologies, however,
sometimes we face unexpected situations, and in this article we present a clinical case. The patient
67 years old with a long history untreated arterial hypertension was revealed congenital defect of the abdominal
aorta and its branches (atypical coarctation of the aorta at the level of the renal arteries and hypoplasia
of the orifices of the visceral arteries), which usually debuts no later than the second decade of life. To detect
this defect allowed resulting from prolonged existence of narrowing of the aorta, a complication of the disease
– poststenotic aneurysm of the abdominal aorta and the clinical manifestations of chronic ischemia of
digestive organs. Intraoperative picture allowed to confirm the congenital origin of changes of the aorta and
visceral arteries. Performing open surgical intervention with radical elimination of the pathological changes
of the aorta and its branches allowed in addition to eliminating the risk of aneurysm rupture to the normalization
of blood pressure and relief of symptoms of chronic ischemia of digestive organs.
