Abstract
The review is concerning to one of the difficult problems of pediatric cardiac surgery - correction of congenital
aortic valve stenosis. One of the methods of surgical treatment of this pathology is the open commissurotomy
(OC). The literature review testifies to the intensive development of criteria for the diagnosis of
congenital AV stenosis based on the cusp morphology and patient selection for the procedure. Despite good
immediate results of commissurotomy in long term may result in AV failure. Thus, the detailed assessment
of the valve and the study of its function before surgical correction is crucial to determine the indications for
surgery. The indications for open commissurotomy and the results of interventions, mainly in children of
early age are presented in the modern literature. Papers devoted to the results of such procedure in older children
are presented mainly by individual studies. However, procedure is positioned as needed, because it
allows to preserve native AV as long as possible, with subsequent adjustments at older age, including valve
replacement. This approach is undoubtedly necessary because it allows to improve significantly quality of life
of the patients' with AV pathology.
