Abstract
The neo-aortic valve regurgitation is a recognized complication late after arterial switch operation in patients
with transposition of the great arteries. Access to the neo-aortic root and neo-aortic valve in these patients is
difficult because of the front position of the pulmonary artery trunk after Lecompte maneuver. The incision
of the ascending aorta below the pulmonary artery bifurcation does not always provide a satisfactory exposition
of the neo-aortic valve. The goal of this paper is a case of successful neo-aortic valve replacement in a
patient in 8 years after arterial switch operation. The approach to the neo-aortic root was performed by
means of the main pulmonary artery cross-section at the anastomosis site to provide good neo-aortic valve
exposition and adequate correction of the regurgitation.
