Abstract
Department of X-ray endovascular and intraoperative methods of diagnostics and treatment of cardiovascular diseases
is one of the units of the Bakoulev National Scientific Center for Cardiovascular Surgery, engaged in X-ray
endovascular diagnosis and treatment of cardiovascular diseases, including in the operating room and in the intensive
care unit.
The purpose of the publication is to present the results of the medical and scientific work of the department for 2016
and to demonstrate main scientific achievements.
During the reporting period, the department carried out a large scientific and practical work on invasive examination
and endovascular treatment of cardiovascular pathology. Four staff members of the department were examined
and treated 3169 patients in the conditions of a single stationary X-ray and two mobile angiocardiographic units.
Diagnostic exams were performed in 2167, and various endovascular interventions were performed in 1002 patients.
In 2016, the number of diagnostic studies increased significantly compared with 2015. The number of coronary
angiography has increased from 465 to 978, angiocardiography in patients with congenitial heart defects (CHD)
from 227 to 322. The number of patients examined in the operating unit and the intensive care unit from 606 to
868. The department pays much attention to the diagnosis and treatment of patients with CHD. It should be noted
that during the reporting period, almost every second (45.2%) of the examined patient with CHD was at the age of
1 year.
In 2016, 1.021 different endovascular interventions were performed in 1002 patients. Of these, planned operations
were carried out in 58.9%, emergency operations in 34.4% and hybrid interventions. Of 1.021 patients, in 180 were
performed percutaneos coronary intervention, in 715 were performed endovascular operations with CHD, 14
stenting of peripheral arteries, 112 other interventions (pericardial catheterization, intra-aortic counterpulsation
balloon installation, etc.). With in the framework of round-the-clock emergency care, the department performed
various endovascular interventions in ishemic heart desease (IHD) (n=87) and CHD (n=158).
One of the most important tasks of the department is development and introduction into clinical practice of hybrid
methods of treatment for patients with cardiovascular pathology, as well as providing X-ray endovascular support for
performing cardiosurgery operations, as well as research in conditions of intensive care unit. In 2016, various hybrid
operations in patients with CHD were performed in 64 cases, and in 5 cases in patients with IHD.
The analysis of intraoperative bypass angiography performed to 816 patients who were subjected to 1975 coronary
bypass. Improved coronary artery bypass grafting, compared with 2015, was noted. The number of dysfunctions in
superimposed conduits decreased from 15.7% (195 cases) to 8.4% (165 cases). In 101 of 165 cases, the revealed dysfunctions
were successfully eliminated intraoperatively. At the end of the operation, 96.9% of the shunts were completely
passable, whereas in 2015, the number of functioning shunts was 90.4%.
During the reporting period, a number of successful rare and complex endovascular interventions in the pathology
of the heart and blood vessels were performed (coronary cameral fistula closure, para-prothetic fistula closure, pulmonary
arterio-venous malformations closure).
