Abstract
Aim of the study: to represent the results of open aortic valve comissurotomy in comparence with balloon
valvuloplasty in older children with congenital aortic valve (AV) stenosis.
Material and methods. From 2000 till 2017, 138 patients underwent AV procedures for congenital AV stenosis.
All patients were divided into two groups: 1st – open aortic comissurotomy (n=61), 2nd – transluminal
balloon valvuloplasty (n=77). Mean age was 8.9±4.2 years in the 1st group, and 9.8±4.0 years in th 2nd.
Mean peak systolic gradient was 83.0±21.0 and 98.9±26.4 mm Hg respectively.
Results. There was no mortality in the 1st group. Non-lethal complications (36%) were pericarditis
(6 patients) and pleurisy (16 patients). Peak gradient at discharge was 21.2±18.0 mm Hg. Three patients
needed AV replacement within 4.8±1.6 years.
There was one in-hospital death in the 2nd group from acute heart failure. Non lethal complications (7.8%)
were common femoral artery thrombosis (5 patients), left anterior descending artery hematoma (1 patient).
Mean peak gradient at discharge was 43.1±15.6 mm Hg. 7 patients needed reoperation within 2 to 7 eyars:
balloon valvuloplasty – 3 patients, AV replacement – 2 patients, AV comissurotomy – 1 patient, and Ross
procedure – 1 patient.
Conclusion. Both technics demonstrate good clinical outcome and are necessary to have in the arsenal of
medical care in patient with congenital AV stenosis. Open aortic valve surgery is accompanied by no mortality
and less re-do surgeries.
