Abstract
True left ventricular aneurysm is a serious complication in the chronic phase after acute myocardial infarction; may be asymptomatic; however, it can cause refractory heart failure, persistent ventricular arrhythmias, and arterial embolism. The main indications for aneurysmectomy and reconstruction of the left ventricle are: anterior-septal, anterior and anterior-lateral infarction with the formation of left ventricular aneurysm; satisfactory contractility of the basal segment of left ventricle; the presence of a thrombus in the left ventricle cavity. Surgical treatment of postinfarction left ventricular aneurysm is aimed at restoring the correct elliptical shape of the heart (forming the apex of the left ventricle), normalizing the diastolic volume of the heart to physiological values, and eliminating areas of asynergy that can cause life-threatening arrhythmias. We report a case of surgical reconstruction of the left ventricle in a patient with post-infarction left ventricular aneurysm.
