Abstract
Valvular heart diseases (VHD) bring significant contribution to cardiovascular morbidity and mortality. Valvular heart diseases have a long asymptomatic period when the body is capable of coping with the increased load on the cardiovascular system. Decompensation of VHD manifests itself as a symptomatology of cardiovascular failure, which is the reason for contacting a cardiologist. On this stage of disease surgical treatment is often less effective, in some cases accompanied by a higher risk of adverse events in the perioperative period.
Assessment of contractile reserve by stress echocardiography is a perspective and rapidly developing area of ultrasound diagnostics. Measurement of the contractile reserve by the speckle tracking technique (global longitudinal
strain measurement) allows to evaluate the presence or absence of the contractile reserve in asymptomatic patients.
Thus, the range of indications for surgical correction of valvular pathology is expanding. Assessment of contractile reserve can be used to resolve controversial issues in indications for valve surgery, early diagnosis in asymptomatic patients, and risk stratification.
