Abstract
Multifocal atherosclerosis is the most common disease of the cardiovascular system, which affects all vessel's pools. Often there is a simultaneous affection of the middle and large arteries in different places. Not rarely there are severe pathology of the coronary arteries, which needs to perform aortocoronary bypass and significant stenosis of the carotid arteries, requiring revascularization. When carotid artery pathology is asymptomatic, the optimal management of the treating these patients with combined coronary artery and carotid artery disease remains controversial. One-stage surgery has shown a higher risk of complications and worse clinical outcomes. Thus, step-by-step surgical treatment is preferable to one-stage surgeries. This clinical case is presented demonstrating the use of ultrasound duplex angioscanning, angiography of carotid arteries, coronarography, which allowed to develop the optimal variant and stage of the surgical intervention. The adopted tactics ensured surgical protection of the brain by means of step-by-step repair of permeability of brachiocephalic and coronary arteries, reduction of risks of stroke in early postoperative period.
