Abstract
Psoriasis is a systemic disease which manifests as an erythematous, flaky rash on the skin, affecting all age groups of patients. Many studies have proven the relationship between psoriasis and diseases of the cardiovascular system, namely atherosclerosis and coronary heart disease. In this clinical case, a young patient with a severe form of psoriasis and a complicated form of ischemic heartdisease is presented. It is noteworthy to mention a decreased left ventricular ejection fraction according to the echocardiography, as well as an occlusion of the left anterior descending artery and severe stenosis of the marginal branch of the circumflex artery. It was decided to perform an operation of coronary artery bypass grafting to this patient. A significant psoriatic lesion of the lower extremities made it difficult to harvest the great saphenous vein as a conduit during coronary artery bypass grafting. For this reason, it was decided to perform bypass surgery using the left internal thoracic artery and the radial artery as conduits.
