Abstract
Functional mitral insufficiency is a disease of the entire mitral-papillary complex involving the left ventricle, therefore, appropriate correction is necessary. That's why, it is not surprising that most of the isolated techniques show unsatisfactory results. It is not surprising that most isolated techniques demonstrate unsatisfactory results, affecting only one component of functional mitral insufficiency, mainly the annulus fibrosus; and, having eliminated valve dilation, does not interfere with the ongoing process of left ventricular remodeling, which leads to the failure of most reconstructions. The article presents a case of a successful multicomponent reconstructive operation on the mitral valve with an effect on the entire mitral-papillary complex in a patient with type IIIb ischemic mitral insufficiency according to A. Carpentier classification. One year after the operation, the optimal hemodynamic parameters on the mitral valve were noted and effective reverse remodeling of the left ventricle was revealed.
