Abstract
The aim of this study was to discuss a rare clinical case of a patient with asymmetric hypertrophic cardiomyopathy combined with tachyform of atrial fibrillation, severe mitral regurgitation, congenital atrial and ventricular septal defect, ischemic heart disease. The article explains the mechanism how hypertension influence on the localization of the myocardial hypertrophy in the presence of hypertrophic cardiopathy phenotype, the influence of comorbid pathology and cardiovascular risk factors on cardiac remodeling and hemodynamics. We present all the steps of diagnosis making, the hybrid surgical treatment tactics, which led to clinical, echocardiographic and hemodynamic improvement. The collaboration of cardiologists and cardiosurgeons ensured the effective and the best tactic treatment.
