Abstract
The purpose of the study – is to examine the diastolic function in patients with coronary artery disease (CAD)
and metabolic syndrome.
Material and methods. Surveyed 103 patients with ischemic heart disease, received hospital treatment in clinical-
diagnostic department of Bakoulev National Medical Research Center for Cardiovascular Surgery.
All patients with CAD were divided into several groups depending on whether they have metabolic syndrome.
In Group 1, all patients had normal body weight (Body Mass Index (BMI) 21.8±1.3), 93% of them
had arterial hypertension (AH) and 46% of diabetes mellitus (DM). In Group 2 patients with overweight
(BMI 29.6±0.1) AH was 96%, DM – 60%; in Group 3 with I degree fat desorder (BMI 32.7±0.2), AH –
89%, DM – 66%; in Group 4, with II degree (BMI 38.8±0.6) AH – 83% and DM – 83%. Diastolic function
was assessed according to the recent consensus guidelines on diastolic function evaluation measuring
mitral inflow velocities (E-wave, A wave, E/A ratio) and deceleration time (DT) using pulsed-wave (PW)
Doppler in the apical four-chamber view.
Results. Patients with normal BMI in 26% of cases there is no violation of myocardial relaxation when overweight
and I degree obesity also has patients with normal diastolic function (20 and 22% respectively). In the
obesity II grade nobody has normal relaxation of the myocardium. With the increase in frequency of occurrence
increases obesity diastolic dysfunction is more severe. More than half the cases, there has been a violation
of the diastolic dysfunction II degree (pseudo normal) and when compared to a group of patients
with a normal body weight received statistical accuracy (p=0.008), with I group (p=0.01) and group II
(p=0.008).
Conclusion. In patients with ischemic heart disease and metabolic syndrome increases with the degree of
severity of obesity and the prevalence of diastolic dysfunction.
