Abstract
Barlow's disease is the most severe form of degenerative mitral valve disease, due to the fact that various valve structures
and segments are affected. Surgical repair of Barlow's disease remains challenging because of the difficulty of the bi-leaflet repair and the risk of systolic anterior motion (SAM). Various variants of mitral valve reconstruction
have been proposed, such as implantation of artificial neochordae, partial resection of the posterior leaflet.
This report presents a case of successful multicomponent reconstruction of the mitral valve with resection of the body
of the posterior mitral leaflet, translocation of the chords from the posterior mitral leaflet to the anterior at the site,
suturing the fissure and annuloplasty on the support ring with a lesion of Barlow. The immediate result of the operation
is characterized by optimal hemodynamics of the mitral valve, satisfactory function of the myocardium of the
left ventricle.
The selected tactics of surgical treatment can be recommended for the correction of this pathology
