Abstract
Objective: to assess the quality of life (QoL) according to SF-36 in the elderly in the medium-long term after correction of valvular heart diseases with/without coronary heart disease (VHD±CHD).
Material and methods. From January 2019 to December 2021, 240 patients ≥ 60 years old (the average age was 69.2 ± 1.3 years) underwent surgical correction of VHA±CHD. According to the Edmonton frail scale/index (EFI) before surgery, the average score was 7.5±1.9 (“vulnerable»). Average indicators of SF-36: physical health (PH) – 50.9±1.0, physical functioning (PF) – 47.9±2.4, role-playing physical functioning (RPF) – 53±2.0, pain – 44.6±3.3, mental health (MH) – 46.3±7.0, role-playing emotional functioning (REF) – 50±2.2, social health (SH) – 58.6±3.7, viability (V) – 64±7.6, general health (GH) – 48.6±3.0. Thus, in all categories except for housing, d/o, low QoL indicators were obtained (<50 points).
Results. The medium-term results of surgical correction of VHD±CAD were evaluated in 209 patients. The average EFI is 3.2±2.9 (“not frail”). Mean SF-36: PH – 55.7±1.9, PF – 58.6±2.7, RPF – 59.3±3.0, pain – 51.6±1.9, MH – 49.6±5.5, REF – 52±3.4, SH – 65.3±2.0, V – 70.2±8.0, GH – 58.5±1.9 (p<0.05). Thus, within 6–24 months after the correction of VHD±CAD, the elderly showed good QoL in terms of physical health components, however, in terms of pain and mental components, the indices remained < 50 points (satisfactory QoL).
Conclusion. Analysis of the medium-long-term results of surgical correction of VHD±CHD with an assessment of QoL within 6–24 months after operations in the elderly showed that the correction of VHD leads to an improvement in the physiological reserve of patients (an increase in the number of “not frail” patients according to EFI) and an improvement in the quality of life of patients within 6–24 months after operations, especially in physical components of health
