Abstract
Objective – study and analysis of changes in temporal and spectral indicators of heart rate variability and the frequency of occurrence of arrhythmias with hyper- and hypothyroidism, along with arterial hypertension and ischemic heart disease.
Materials and methods. An analysis was made of heart rate variability and the incidence of arrhythmias among 101 patients with impaired thyroid function along with ischemic heart disease and arterial hypertension. The results of clinical, laboratory and instrumental examinations obtained during the observation of these patients are analyzed. To determine changes in heart rate variability and the incidence of arrhythmias in the examined patients compared with the control group and in combination with each other, a statistical analysis was performed.
Results. Reflecting heart rate variability in general, SDNN decreased significantly in both groups compared with the control group (p<0.001). HF.nu values characterizing parasympathetic influences on the heart rhythm in patients with hypothyroidism were higher than in the control group (p=0.002). The number of supraventricular extrasystoles in patients with hypothyroidism increased 2 times than in the hyperthyroid group and exceeded the values of the control group by 287 times (p<0.001). In the group with hyperthyroidism, the frequency of ventricular extrasystoles was 8.5 times higher compared with hypothyroidism, which was statistically unreliable (p=0.394) and exceeded the values of the control group by 20.1 times (p=0.003).
Conclusion. Identified predictors determine the need for an integrated approach to dynamic observation, correction of cardiological endocrinological tactics in the treatment of patients with this pathology.
