Abstract
Cases of detecting a functioning patent ductus arteriosus of hemodynamically significant sizes in adults with an existing critical aortic stenosis are rare. In the world literature only 11 cases of successful treatment of such patients are reported. Cardiac surgery in an elderly patient with patent ductus arteriosus causes certain difficulties associated with a high operational and anesthetic risk, the presence of age-related changes in the walls of the great vessels, the development of high pulmonary hypertension and the lack of clear clinical guidelines. The article describes a clinical case of treatment of an elderly patient with a newly diagnosed functioning patent ductus arteriosus with a diameter of 5.6 mm and critical stenosis of the aortic valve by transcatheter closure of the ductus arteriosus with an 11 mm PDA occlusion device and replacement of the aortic valve with a mechanical prosthesis Carbonix No. 22. Staged surgical treatment made it possible to level the risks of possible complications, to achieve a positive clinical and hemodynamic result.
