Abstract
The formation of a postinfarction left ventricular aneurysm is one of the most common and dangerous mechanical complications of acute myocardial infarction. Its main localization is the anterior wall, interventricular septum and the apex of the left ventricle, and the involvement of the lateral wall in the pathological process of aneurysm formation is considered quite rare. This paper describes our experience of surgical treatment of the patient a 74-year-old patient with a large-sized thrombosed postinfarction left ventricle lateral wall aneurysm with the left ventricle reconstruction on a beating heart. The presented patient revealed a significant decrease in the left ventricular ejection fraction to 28.8%, concomitant pathologies (diabetes mellitus, chronic pyelonephritis), as well as a condition after COVID infection, which could contribute to the development of complications during the intervention and postoperative period, but all stages of treatment proceeded smoothly and the patient was discharged home in a satisfactory condition in the accepted deadlines.
